Reimagining Care for Older Adults in WNC

WNC Bridge Foundation continues our support of MAHEC’s Home Based Primary Care (HBPC) program with a second IMPACT Grant of $100,000, awarded to the organization in 2020. MAHEC’s innovative program reimagines how care of older adults is delivered in Western North Carolina and the type of care they receive. We are also supporting the YMCA of WNC with a grant to help improve older adults’ health, and one element of this grant is to provide fresh, mostly local produce to families. Many of these clients are from the HBPC program.


Recently, a box containing fresh fruit and vegetables, including celery, was delivered to an HBPC participant and her daughter. The recipient shared with a smile, “Oh celery! I thought about buying this yesterday, but it was too expensive!”


Being able to deliver nutritious food into the hands of those in need is only one of the many ways the Home Based Primary Care program is making a difference in the lives of the elderly in Western North Carolina. WNC Bridge Foundation also acknowledges the work of two of our grantees working together.
